A Clinical Study on 261 Cases of Colorectal Cancer



Abstract
In Korea, the rate of incidence and mortality of colorectal cancer has been increased, this report is retrospective review of colorectal cancer in 261 cases followup from January 1981 to December 1993 who were admitted and operation was done.
These patients were all confirmed by pathological diagnosis at Department of General Sergery, Taeioen Eulji General Hospital
@ES the results were as follow:
@EN 1) the ratio beteen male and female was 1.27 : 1 and peak incidence was 7the decade but differenciation was seen at 1980's and 1990's 1980's peak age distribution was 6th, 7th Decade but 1990's peak age ditribution was 5th, 6th, 7th decade
2) the more frequent symptoms and signs were abdominal pain(86.7%), weight loss(31.7%) and palpable mass(20.0%) in the rt. colon cancer, abdominal pain(74.0%), bloody stool(42.0%), bowel habit change(36.0%) in the It. Colon cancer Melena or
bloody
stool(84.1%), bowel habit change(54.3%), tenesmus(51.7%) in the rectal cancer.
3) 91.6% of rectal cancer was diagnosed initially by digital examination. 94.4% of rectal cancer and 95.3% of colon cancer were diagnosed by colon study. 79.4% of rectal cancer and 93.1% of colon cancer were diagnosed by colonofiberoscope and
sigmoidofiberoscope. 98.4% of rectal cancer and 95.7% of colon cancer were diagnosed by C-T scanning.
4) The duration of the symmptoms and signs prior to admission below 3 months was most common 41.7%
5) location of the tumor was the most frequent in the rectum(57.9%), sigmoid colon(14.6%), ascending colon(6.9%).
6) radical cancer surgery was done 192 cases(73.5%) and palliative surgery was done 69 cases(26.5%) at radical cancer surgery miles' operation was most frequently performed 94 cases(36.1%).
7) The survival rate of at radical cancer surgery 1yr 88.5%, 2yr 73.3%, 3yr 58.2%, 4yr 47.6%, 5yr 41.3% at palliative surgery 6month 63.7%, 1yr 44.9%, 2yr 5.8% and mean survival month were 14.8 months
8) the most frequent macroscopic finding of colorectal cancer was annular type 54.4%.
9) the most frequent histopathological type was adenocarcinoma 98.8%
10) the most common metastatic site was pelvic organ 16.1%.
11) the stage of colorectal cancer were modified Dukes classification A in 1.1%, B in 29.2%, C in 42.9%, D in 26.8%
12) postoperative complication were developed in 27.9% of total cases, wound complication was most frequent 10.7%
13) operative mortality was 1.9%
